Ergonomic Assessment

If you would like an assessment completed, complete the request, and when submitted, the Employee Health and Wellness Office will make an appointment for the assessment.

When the assessment is completed, recommendations will be provided to the supervisor and the employee. Dependent on the recommendations and the supervisor’s decision to implement, the Employee Health and Wellness Office will assist in the coordination to implement the recommendations. If eligible, the costs for ergonomic supplies/equipment will be split between the Employee Health and Wellness Office and the Requesting Department. A request to Purchasing for pricing will then be made to begin the process to purchase as appropriate.
